NEW EMPLOYEE INFORMATION

The I-9 needs to be completed by the employee and related documents shown to you within 3 business days.  You can find the I-9 here  www.uscis.gov/files/form/I-9.pdf

The W-4 needs to be completed by the employee so that employers know how much federal tax to withhold.  You can find this form here www.irs.gov/pub/irs-pdf/fw4.pdf

For employees working in Louisiana, they need to fill out an L-4 which lets employers know how much Louisiana state tax to withhold. You can find this form here L-4

EMPLOYEE TERMINATION

As a reminder, you should ask your employees to update their address and status annually.  If their filing status has been changed they should fill out new W-4s and L-4s. 

Annual Bonuses should be treated as payroll checks so that taxes are withheld and reflected on the employee's W2 at year end.

For Contract workers, upon hiring they should fill out a W-9 www.irs.gov/pub/irs-pdf/fw9.pdf  or W-9.
